Thank you for allowing us to be your health care practice where we strive for excellence and consider finding the cause of your symptoms our top priority, not just putting a band aid on them. Because this office works a bit differently, our new patient paperwork is quite extensive. Why? We must have the full gamut of information to start our research into your case. It is not as simple as stating your symptom, and then us giving you a prescription to “cover up” the root cause.
With that said, you must have your new patient paperwork with you at the time of your appointment or sent to the office beforehand. This will allow the full amount of time for your new patient appointment rather than having to shorten it due to paperwork needing to be filled out.
Along with your paperwork please bring your insurance card, driver’s license, the past 12 months of medical records that are available, all supplement and rx bottles.
Your new patient appointment will be three fold: A lab test called a microscopy which gives us insight to your systemic environment. A new patient assessment and planning session with Dr. Gonino, and the new patient wellness class which is the foundation of our program explaining the integrative approach to medicine along with reasons that lifestyle changes are necessary for proper healing. The success of our program is due to patients attending this class and Dr. Gonino prefers you attend within the first week of your new patient appointment.

If this is the first time, please use "New Sign Up". A link will be sent to your email and from that link you will be able to create a password and then sign on to the Portal.
The portal will provide you access to your labs and your treatment plans. If a prescription is involved in your lab workup from the doctor, a staff member will be calling you, otherwise all recommendations and notes will be with your labs in your portal.
If you cannot access your portal for any reason the NEW PATIENT packet below can be printed and returned to the office at NPcoordinator@goninowellness.com before your first appointment.
